Click here to download our Medical Release form. A form should be completed for each medical entity having medical records pertinent to the reason for your visit with our physician. The forms should be returned to our office as soon as possible to assure that the medical records are secured prior to your visit. Appointments may be rescheduled if the records are not received prior to your appointment date.
